The Duality Session – Akashic Soul Alchemy Reading - Astral Reiki™

Client Consent & Understanding Form

Provided by: Quantum Integration Station LLC and Your Abundant Design LLC (“we,” or “us”) for services delivered under The Duality Session collaboration.

By signing this form, you acknowledge and agree to the following terms regarding your participation in the Akashic Soul Alchemy Reading and Astral Reiki™ aspects of The Duality Session.

1. Nature of the Session

The Akashic Soul Alchemy Reading and Astral Reiki™ aspects of The Duality Session is intended for personal growth, spiritual exploration, and entertainment purposes only. The session is based on the practitioners' intuitive guidance and energetic insights, which are interpreted and shared in alignment with the client's spiritual path.

Please understand that the Akashic Soul Alchemy Reading and Astral Reiki™ aspects of The Duality Session are not scientifically proven and are not a substitute for professional advice. Any actions you take based on the session’s guidance are entirely your choice and responsibility. For legal, medical, financial, tax, or business matters, you are responsible for consulting licensed professionals before making any decisions.

2. Practitioner Role & Medical Disclaimer

The Quantum Integration Station LLC practitioner collaborating in The Duality Session is a licensed healthcare professional, but the services offered during this session do not constitute medical diagnosis, treatment, or care.

This session is designed to support personal growth, energetic balance, and spiritual exploration, and is provided as a complementary service outside the scope of any medical treatment /licensure the practitioner may provide as a licensed professional.

The practitioners do not provide medical advice, prescriptions, or substitutes for medical or mental health care during this session. Participation does not replace or override ongoing medical or mental health treatment by your licensed healthcare provider(s).

3. Release of Liability

By consenting to this session, you acknowledge and agree that:

• I am fully responsible for my health, well-being, and actions during and after the session.

• The practitioners, The Duality Session, Quantum Integration Station LLC, and Your Abundant Design LLC are forever released from any liability or claims arising from medical, psychological, or physical health issues related to or resulting from participation in this session.

• Any actions, decisions, or outcomes resulting from guidance, recommendations, or information provided during the Akashic Soul Alchemy Reading and Astral Reiki™ aspects of The Duality Session are solely my responsibility. I hereby forever release the practitioners, The Duality Session, Your Abundant Design LLC, and Quantum Integration Station LLC from any and all liability, claims, or legal actions arising from my voluntary participation.

• I further release the practitioners, The Duality Session, Your Abundant Design LLC, and Quantum Integration Station LLC from any actions, claims, or demands that I, my heirs, or my legal representatives may have, both now and in the future, related to my participation in a Akashic Soul Alchemy Reading and Astral Reiki release the practitioners, The Duality Session, Your Abundant Design LLC, and Quantum Integration Station LLC aspects of The Duality Session. This includes, but is not limited to, injury, death, or property damage, whether or not these actions are connected to the session.

• In the event of a legal dispute, I agree to assume responsibility for attorney fees for all parties involved.

• I understand that participation in the session does not establish a therapist-client relationship.

4. Ceasing the Session

The practitioners may cease the session at any time if necessary. If stopped, the practitioner will endeavor to explain the reason but is not legally obligated to do so.
You are responsible for disclosing any psychological or emotional issues prior to the session. The decision to proceed is at the practitioner’s discretion, ensuring you are mentally and emotionally prepared.

5. Confidentiality

All information shared during The Duality Session is confidential. Disclosure to others is your choice.

6. Age Restrictions

Services are only available to adults 18 years or older.

7. Mental Capacity & Voluntary Participation

I affirm that I am of sound mind, mentally competent, and fully capable of making my own decisions regarding participation. I am voluntarily participating without undue influence, coercion, or misrepresentation.

8. Separation of Liability Between Entities

I understand that:

• Quantum Integration Station LLC, Your Abundant Design LLC, and The Duality Session all operate as separate legal entities.

• I understand and agree that any legal action, claim, or dispute against The Duality Session cannot be directed toward Quantum Integration Station LLC, Your Abundant Design LLC or its practitioners, and vice versa.

• I understand and agree that participation in services provided by one entity does not create liability or responsibility for the other entity.

9. Participation & Suggested Practices

Any recommended practices—meditation, reading materials, or exercises—are optional. I may choose whether to engage, and assume responsibility for any outcomes.

10. Accuracy of Information

I represent that all information provided to the practitioners, Quantum Integration Station LLC, Your Abundant Design LLC, and The Duality Session is true and accurate. It is my responsibility to disclose any updated pertinent information in writing and verbally to the practitioners prior to the session. I agree to hold the practitioners, Quantum Integration Station LLC, Your Abundant Design LLC, and The Duality Session harmless from liability arising from the lawful disclosure or use of such information.

11. Cancellations, Rescheduling & Refunds

• Cancellations require a minimum of 24 hours’ notice. Less than 24 hours’ notice will result in responsibility for the full session fee.

• Payments for each session are non-refundable

• Upfront payment secures your appointment; confirmation depends on successful processing of payment.

• Exceptions to this policy are considered case-by-case and do not set precedent.

12. Privacy & Data Sharing

I authorize the practitioner(s) of Quantum Integration Station LLC acting on behalf of The Duality Session to collect personal and medical information for session purposes. I am responsible for updating any relevant health information prior to the session and relaying this before The Duality Session begins. In emergencies, the practitioner(s) of Quantum Integration Station LLC acting in collaboration with the practitioner(s) of Your Abundant Design LLC on behalf of The Duality Session may communicate relevant information to authorities or healthcare providers at its discretion.

I understand that my information is confidential, and will only be shared with third parties with my written permission. Information I provide may include my name, email, and other details submitted through forms, booking, or communication. The Duality Session uses this information to:

• Provide requested services

• Process appointments

• Communicate regarding updates, promotions, or educational content

The Duality Session may collect non-identifying data automatically through server logs or analytics (e.g., IP address, browser type, pages visited). The Duality Session does not sell, rent, or lease your personal information to third parties.

Emails comply with the CAN-SPAM Act, including identification of the sender, a valid reply-to address, and an option to unsubscribe.

While professional standards of confidentiality are maintained, any communication, including emails or forms submitted via the website, (The Duality Session Page) may be subject to email security limitations.

13. Limitation of Liability

To the fullest extent permitted by law, The Duality Session, Quantum Integration Station LLC, Your Abundant Design LLC, and their practitioners shall not be liable for any direct, indirect, incidental, consequential, or special damages resulting from participation in services.

14. Agreement & Confirmation

In purchasing The Duality Session, I confirm that I:

• Have read, understood, and agree to the terms and conditions of this Consent & Understanding Form.

• Accept full personal responsibility for my participation.

• Affirm that I am mentally competent and voluntarily participating.

• Agree to forever release The Duality Session, Quantum Integration Station LLC, Your Abundant Design LLC, and the practitioners from any liability or claims.

• Understand the separation of liability among the three entities.

• Agree to the policies for cancellations, payments, and session participation.
